Transportation Agreement (School Age Children)
By my signature below, I am authorizing Messiah Lutheran Preschool and Kindergarten (MLPK)
to escort my child (print name)

to and/or from MLPK’s Before and After School Care program to and/or from bus
transportation provided by Wauconda Community Unit School District #118. I understand that
the responsibility for the care and safety of my child is transferred to the school district as soon
as my child boards the bus and continues until the child is received off the bus at MLPK. Please
check the appropriate boxes below.

I understand that I am required to notify MLPK if my child is not attending school. MLPK will
notify me if my child is not received from the District #118 bus as expected.
