RECESS Waiver and Release of Liability

I hereby give my permission for my child _____________________ to participate in RECESS

programs and activities.

I am aware of the risks connected with attending RECESS. I agree to not hold accountable or

bring legal action against RECESS LLC or Matt Nielsen. This waiver releases the company from

any and all liability relating to injuries that may occur at any RECESS Pop-Up or Class in Tampa,

Florida before, during, or after any and all camp sessions.

By signing this agreement I agree to hold the company entirely free and harmless from any

liability, including financial responsibility for injuries incurred, regardless of the reasons or

circumstances including without limitation the actions or inactions of the other camp particpants.

By signing below, I acknowledge the risk presented myself and my child involving this activity. I

certify that our participation is voluntary and we have been made aware of the risks.

RECESS Media Release

I, being the parent/guardian of _______________________________ hereby consent that

photographs and video may be taken of my child while he/she is enrolled at RECESS.

These pictures may be used on RECESS social media, RECESS websites, RECESS emails, etc.



I give my permission to use my son/daughter’s photograph, etc. as described above.



I DO NOT give my consent to have photographs or videos of my son/daughter used by

Camp Classroom in any way.
