FORMS – ALL STUDENT MUST COMPLETE EVERY YEAR
DIRECTIONS: Complete the following documents and submit electronically or pick up a paper packet or print the packet and
complete by hand and turn them in during Von Business Days the week prior to the first day of school or turn them into the
Student Support Office Room 110. Make sure to complete all of the MANDATORY documents that are listed below.

● To apply, you must check YES on the Family Income form and Qualify for Free/Reduced Lunch
● Emergency Contact Form ( if a person is not listed, they can NOT pick up a student for early dismissal)
● Robo Call Consent
● Media Consent
● Medical Info form
● Race\Ethnicity Survey
● Medical Compliance:
● Health Physical Due 9

DIRECTIONS: Please bring the following documents to Von Business Days or turn than into the Student Support Office Room 110

● COPY Student Birth Certificate
● COPY of TWO Forms of Proof of Address
● COPY of Guardian ID
PREPARING FOR VON REGISTRATION \ BUSINESS DAYS
Student Fees - $250
- Includes things such as: class materials\handouts, Loaning Chromebook, First Student ID, Yearbook, Field Trip Expenses,
Textbooks, and more
- Can pay in person with Cash, Cashier Check, Money Order
- Can Pay with Credit Card or Debit Card via Parent Portal ( CLICK HERE )
PE Uniforms
- All Freshmen are automatically charged an additional $25 for a PE Shirt, Shorts and Lock
- This can be paid with CASH In-Person or by Credit Card or Debit Card via Parent Portal
- Additional PE Uniforms can be purchased for all Grade Level as follows:
- T-Shirt $8
- Shorts $10
- Hoodie $35
- Sweatpants $20
- Lock $7
Tech Fee
- Each Student is provided a Chromebook to loan the 4 years they are at Von Steuben, it is their responsibility to take care of
the Chromebook and make sure it is not lost, stolen or damaged.
- As long as the Chromebook and Charger is returned in the same condition it was given there will be no charge to the student \
family
- If the Chromebook and\or Charger is lost, stolen or damaged then there will be a $250 charge to replace the Chromebook
and $20 Charge to replace the Charger .
Student IDs
- Students are provided their First Student ID, as a part of their Student Fees & must wear it everyday
- Each day, a student arrives to school without their ID, they will be charged $1 for a Temporary ID
- If a Student loses their ID , they must purchase a new ID for $5
