DDDS Waiver Long Term Care Benefits
Individuals enrolled in this benefit receive case management from a qualified
MCO case manager.
Individuals enrolled in this benefit receive case management from a qualified
DDDS case manager.
Community-based residential alternatives that include Assisted Living Facilities
Residential Habilitation in the following settings:
• Neighborhood Group Home (agency-only)
• Community Living Arrangement Staffed Apartment (agency- only)
• Shared Living (non-agency)
Non-Residential Services available to individuals residing in a nursing facility
or assisted living facility.
Specialized durable medical equipment and supplies not covered under the
Medicaid State Plan. This service includes: (a) devices, controls, or
appliances specified in the plan of care that enable the member to increase
his/her ability to perform activities of daily living; (b) devices, controls, or
appliances that enable the member to perceive, control, or communicate with
the environment in which he/she lives; (c) items necessary for life support or
to address physical conditions along with ancillary supplies and equipment
necessary to the proper functioning of such items; (d) such other durable and
non-durable medical equipment not available under the State Plan that is
necessary to address participant functional limitations; and, (e) necessary
medical supplies not available under the State Plan. Items reimbursed under
DSHP Plus are in addition to any medical equipment and supplies furnished
under the State Plan and exclude those items that are not of direct medical or
remedial benefit to the member. This service does not duplicate a service
provided under the State Plan as an expanded EPSDT service.
DDDS waiver does not cover.
Transition services for those moving from a nursing facility to the community
under the Money Follows the Person (MFP) program (can include security
deposit, telephone connection fee, groceries, furniture, linens, etc.,).
Limitations: Up to $2,500 per transition, but the MCO case manager may
authorize service request exceptions above this limit.
DDDS waiver does not cover.
