Dance Guest Pass Application & Agreement
Non-EHS students may only attend the following three dances: Homecoming, MORP(Winter Rally)
and Prom
All students & their guests must conduct themselves within the guidelines and policies of Elko High
School, the Elko County School District and abide by the following:
• This form is due in the Main Office no later than Tuesday before dance.
• You may invite one guest only!
• Guests must be at least a 9
th
grader and no older than 20 years. 7
th
& 8
th
graders are not permitted to attend.
• Guests must show photo i.d. from the high school they attend; guests who do not attend high school must show
another form of picture i.d., example: driver’s license.
• Elko High School students are responsible for the behavior & demeanor of their guest – EHS students are
responsible for informing their guest of all EHS behavior & dress code rules. EHS student’s signature below
indicates he/she understands & accepts all behavior policies for himself/herself & his/her guest.
• Students must enter and leave with their guest.
• Refunds will not be issued for failure to comply with the guidelines of Elko High School, the Elko County School
District, or those set forth within this form.
