THIS MODEL AGREEMENT AND RELEASE (“Agreement”) is dated and is between the undersigned and on behalf
of Go Brown Mobile Spray Tanning Company and the undersigned model/performer (“I”).
Agreement is as follows:
For valuable consideration received, I_______________________________ , hereby grant Go Brown and the
photographer,_______________________________ , absolute and irrevocable right and unrestricted permission to use, re-use,
display, transmit, publish, re publish, copy, or otherwise exploit, either in whole or in part, digitally , in print or in any other
medium now or hereafter known, for any purpose whatsoever and without restriction, photographs taken of me, or in which I
was included; to alter the same without restriction, and to copyright the same.
I understand and agree that GO BROWN and the photographer may or may not use my name in conjunction with the
photographs as he or she so chooses.
I further give my irrevocable consent to GO BROWN Directors and his/her direct or indirect licensees and assignees to publish,
republish or otherwise transmit the images of myself in any medium for all purposes throughout the world. I understand that
the images may be altered or modified in any manner.
I hereby waive any right that I may have to inspect and approve a finished product or the copy that may be used in connection
with an image that the Photographer, on behalf of the Directorate of GO BROWN has taken of me, or the use to which it may be
applied.
I further release the Photographer and or the Directorate of GO BROWN and his/her direct or indirect licensees and assignees,
from any claims for remuneration associated with any form of damage, foreseen or unforeseen, associated with the use of the
images or named shoot.
Consent. I represent that I am over the age of eighteen (18) years and that I have read the forgoing and fully understand its
contents. This release shall be binding upon me, my heirs, legal representatives, and assigns.
Entire Agreement. This Agreement contains the entire agreement between the parties and relating to the subject matter hereof
and supersedes any and all prior agreements written or verbal.
