Disclosure Statement
General Information
The State of Colorado Department of Regulatory Agencies regulates the practice of licensed and
unlicensed psychotherapists. If you have any questions, concerns, or complaints regarding our
therapeutic relationship, please let me know so that we can address them immediately. You
always have the right to contact the State Board listed below with concerns:

Client’s Rights
● As a client, you are entitled to receive information about my methods of therapy, the
techniques used, the duration of therapy (if known), and the fee structure. You may
always seek a second opinion from another psychotherapist, psychologist or psychiatrist.
You can terminate therapy at any time.
● In a psychotherapy relationship, sexual intimacy is never appropriate and should be
reported to the Department of Regulatory Agencies – contact info above.
● The personal information provided by you during therapy is legally confidential in the
State of Colorado. I do not keep therapeutic information about you in any electronic
form and will only share such information under the following circumstances:
○ When you give me permission and sign a release of information to another
person or agency
○ When there are signs that you are a danger to yourself or others
○ When there is evidence or danger of child abuse
○ In confidential discussion with a qualified professional supervisor.
You have the right to view your psychotherapy notes, but I do not let any third person
have access to those notes except under legal duress.

Sessions via Zoom or by email:
● May be disrupted or restricted by technological problems/limitations beyond the control
of the therapist
● May not be as comprehensive or therapeutically effective as face-to-face consultations
