● Applications must be postmarked by
● All vendors need to provide their own tents, tables, chairs, etc.you are renting space only. Tents must be weighted down and
securely staked.
● The event area is located on the upper parcel of Crooked Creek Campground around the Event Tent. Please be respectful of
the campers and stay within the marked areas.
● Several music performances will be occurring throughout the day. Any music, sound, or generators must not conflict with a
performance.
● All vendors are expected to participate for the full day schedule on Saturday from 2PM-6PM, no early setups or tear downs.
Food vendors may extend their hours with prior permission. If vendors leave or break down early your application will not be
considered for future events. Booth setup will be on the day of the event starting at 12pm (Noon), unless otherwise requested.
● Food Vendors: Please attach a full menu and price listing.
● Merchandise Vendors: Please attach a picture of the booth display. Give a general description of items for sale, display, and
include price range. Please provide a list of other fairs, events, or events in which you have participated.
● Each booth must provide their own insurance to cover sales, products, and operations OR sign a waiver of liability. Proof of
insurance must be provided one month prior to the event date. Insurance requirements will be as follows:
○ General Liability Aggregate $1,000,000
○ Property Damage $300,000 per occurrence Or $1,000,000 per occurrence combined single limit. DB Adventures
must be named as additionally insured.
● I have reviewed the above policies and procedures and agree to abide by the policies as outlined, including that the event
is held rain or shine and that booth fees are non-refundable.
● I understand as a vendor of the event that I am a representative of the event and will act as an example to the community.
I understand that I am vending at my own risk and I agree to be responsible for my own safety, protection of belongings,
and actions. I further agree to indemnify and hold harmless the Smoke on the Water: Pine Creek organizers, DB
Adventures LLC, City of Gaines, and any persons holding legal title to the lands upon which this event is being held,
from any claims arising from my participation as a vendor in this event.
